The Care Team Hospice is seeking an experienced Clinical Manager (RN) to lead and support our clinical team, focusing on developing clinicians and delivering high-quality end-of-life care. You will partner with branch leadership to oversee daily operations, mentor staff, and ensure compliance with hospice regulations.

Qualifications

• Active RN license required.
• Minimum 2 years RN experience, including at least 1 year in leadership in hospice or home health.
• Experience with Homecare Homebase (HCHB) preferred.
• Strong leadership, communication, and organizational skills.
• Ability to coach teams and manage multiple priorities.
• Valid driver's license with reliable transportation.

Responsibilities

• Lead and supervise daily clinical operations while ensuring compliance with hospice regulations, company policies, and quality standards.
• Mentor, educate, and develop clinical staff through coaching, orientation, documentation review, and ongoing performance support.
• Oversee patient care coordination, care plan reviews, admissions, orders, and interdisciplinary team collaboration to ensure exceptional patient outcomes.
• Monitor quality metrics, conduct chart audits, and support survey readiness and performance improvement initiatives.
• Partner with branch leadership, physicians, referral sources, and operational teams to promote clinical excellence and outstanding patient and family experiences.

The Care Team Hospice is seeking an experienced and compassionate Clinical Manager (RN) to lead and support our clinical team. As a Clinical Manager, you'll partner with branch leadership to oversee daily clinical operations, support patient care coordination, mentor field clinicians, and ensure compliance with hospice regulations and documentation standards. You'll serve as a clinical resource for your team while collaborating with physicians, referral sources, patients, and families to deliver exceptional end-of-life care.
