Job summary

HIA Hospice in Fargo is seeking an experienced RN for weekend triage and patient support. In this role you will triage calls from patients, families, facilities, physicians and staff, assess situations, and coordinate the interdisciplinary group to meet patient needs.

You’ll contact physicians, pharmacies and DME, process medication changes and orders, document interactions thoroughly, and ensure timely communication and compliance with policies.

• RN degree; BSN preferred, LPN also considered.

Responsibilities

• Triage calls from patients/families, facilities/physicians and HIA Hospice staff.
• Assess each situation and determine the best course of action.
• Assist in daily contact with patients who received medication changes.
• Contact physician, pharmacy, and DME company with death notifications.
• Determine appropriate IDG member for critical patient issues and coordinate contact/visit.
• Order DME as requested by staff or after phone assessment.
• Complete medication orders and refill requests.
• Communicate triage updates to team members.
• Complete thorough documentation of calls with follow-up plan details.
• Prints and faxes requested paperwork to facilities for respite stays and NH replacements.
• Obtain attending MD signature when required on papers.
• Fax scripts to pharmacies and other facilities and monitor transmission.
• Complete and lock documentation the same day as contact occurs per policy.

Coordination of the interdisciplinary team staff to meet the needs of the caller.This position is a weekend status, non-exempt position working every Saturday and Sunday 8am - 8pm and will be based out of our Fargo office.Key ResponsibilitiesProvide excellent customer service by triaging calls from patient/families, facilities/physicians and HIA Hospice staff.Assess each situation and determine the best course of action.Assist in the daily contact with patients who received medication changes.Contact physician, pharmacy, and DME company with death notifications.For critical patient issues, determine the appropriate member of the IDG to attend to the patient/family needs and coordinate the contact or visit.Order DME as requested by staff or as a result of phone assessment.Completes medication order & refill requestsCommunicates triage updates to team membersCompletes thorough, accurate documentation of patient phone calls, including follow-up plan details to assist with patient plan of care.Prints and faxes requested paperwork to facilities for respite stays and NH replacements.
