Job summary

HIA Hospice is seeking a Float RN to provide compassionate nursing care to hospice patients, coordinating with the interdisciplinary group (IDG). The position supports the Fargo office and surrounding areas with travel and a 10-hour, 4-day workweek from 8am to 7pm.

Responsibilities include admission processing, initial assessments, ongoing patient/family care planning, and education on caregiving and end-of-life topics. Strong communication and flexibility are essential in this role.

Qualifications

• RN degree required; BSN preferred.
• Minimum of one year of hospital staff nurse experience or two years in health care.
• Strong interpersonal communication, counseling, and teaching skills.
• Flexibility in scheduling; team player.
• Current state RN licensure in North Dakota and Minnesota (willing to obtain dual licensure upon offer).

Responsibilities

• Conducts hospice referrals and accurately completes admission paperwork/consents.
• Conducts hospice initial assessments at the time of admission.
• Assesses patient/family needs; updates care plan with IDG and facility staff as applicable.
• Maintains an up-to-date individualized care plan reflecting ongoing care.
• Completes and locks documentation the same day as contact occurs and follows policy.
• Utilizes physician orders and adheres to Levels of Care policy.
• Educates patient/family in caregiving and end-of-life topics.
• Relays patient care updates to care teams at end of each workday.
• Performs RN duties across multiple teams (triage, referrals, home, hospital, nursing home, color-coded teams).

All nursing care, as approved by patients’ primary physicians or the hospice medical director, including hospice referrals, patient admissions, physical assessments, symptom management and emotional support.This position will support and float from the Fargo office to the surrounding areas. Travel expected. Flexibility is essential to this role, with the typical shift working 10-hours, 4 days per week. (8am - 7pm)Key ResponsibilitiesConducts hospice referrals and accurately completes admission paperwork/consents.Conducts hospice initial assessments at the time of admission.Assesses the patient’s/family’s needs on an ongoing basis while hospice care is being provided through comprehensive assessment updates.Develops a plan on care in collaboration with the patient/family, facility staff as applicable, and the hospice IDG that identifies needs, expected outcomes, and interventions for each patient/family.Maintains an individualized, up to date care plan that reflects the care being provided to patients/families.Completes and locks documentation on the same day as the contact/communication occurs as per the Medical Records Documentation policy.Implements and utilizes physician orders accurately and effectively.Utilizes levels of care appropriately and shows responsibility for adhering to the Levels of Care policy.Teaches patient/family in areas related to caregiving and in preparation for death as reflected in documentation.Patient care updates are relayed to patient care teams at the end of each working day.Completes RN duties for assignments in various teams: triage, referral/admissions, Referral/Admissions Coordinator, Home team, Hospital Team, Nursing Home Team, Green, Blue, and Purple teams.Accurately uses expense reports for mileage charged to other team accounts.Benefits:HIA offers a robust benefits package for qualifying full-time and part-time employees.
