Mercy Health is seeking an Advanced Social Worker in the Oncology physician practices in Ohio. The role focuses on navigating complex physical, mental, and emotional challenges, providing high-level support, advocacy, and evidence-based counseling within a multidisciplinary team.

Responsibilities include assessing needs, planning personalized interventions, crisis response, and collaboration with providers to ensure comprehensive patient care.

Formación

• Master’s degree in Social Work (MSW) is required.
• Licensed as a Social Worker (LSW) in state of practice is required.
• Licensed as a Clinical Social Worker (LCSW) or LISW preferred.
• Minimum 2 years in social or human services with direct practice, assessment and treatment planning.

Responsabilidades

• Evaluates patients’ needs, situations, and support networks to identify goals and develop individualized care plans.
• Develops advanced intervention strategies and treatment plans for complex patient populations.
• Provides emotional support, guidance, education and counseling using evidence-based techniques.
• Manages cases, tracks progress, and refers patients to community resources.
• Represents patients’ interests, advocates for resources, and addresses barriers.
• Responds to crisis situations and connects patients with resources.
• Coordinates care with a multidisciplinary care team.
