Job summary

Med Source Consultants is seeking per diem Family Nurse Practitioners and Physician Assistants to join a primary care team in New Britain, CT, delivering mobile and community-based preventive and urgent care to underserved populations. These roles are 100% outpatient with travel to home-base hubs in Hartford and New Britain, then to clinic sites and Street Medicine locations.

A CT medical license (or eligibility) and 2+ years of FNP/PA experience are required, with a Monday–Friday schedule and

Qualifications

• Must hold an unrestricted State of Connecticut Medical License (or be eligible).
• Minimum 2+ years of experience as a Family Nurse Practitioner or Physician Assistant, preferably in primary care or urgent care.
• Per diem travel to home base hubs in Hartford and New Britain, then to clinic sites and Street Medicine locations.

Responsibilities

• Provide high-quality preventative and urgent care through mobile and community-based services addressing health disparities and promoting wellness.
• 100% outpatient primary care, urgent care, and family medicine setting.
• Perform routine physical examinations and formulate medical, educational, and psychosocial plans to treat patients.
• Develop plans for further evaluation of health needs in consultation with physicians and care team members, including referrals to specialists.
• Provide appropriate treatment for problems within scope of practice and refer outside-of-scope issues appropriately to physicians.

• Provide high-quality preventative and urgent care through mobile and community-based services addressing health disparities and promoting wellness
• 100% outpatient primary care, urgent care, and family medicine setting
• Position requires travel to home base hubs in Hartford and New Britain, then travel via company fleet to clinic sites and Street Medicine locations
• Full-time, weekdays only schedule (Monday through Friday)
• No call required
• Perform routine physical examinations and formulate medical, educational, and psychosocial plans to treat patients
• Develop plans for further evaluation of health needs in consultation with physicians and care team members, including referrals to specialists
• Provide appropriate treatment for problems within scope of practice and refer outside-of-scope issues appropriately to physicians
