STGi is seeking a Nurse Practitioner to provide services at our Georgia War Veterans Home. GWVH will be opening a new unit that will be dedicated to the needs related to diagnoses of Veterans with PTSD, TBI, history of behavioral disorders, and substance use issues. JOB SUMMARY:
The primary purpose of this position is to provide primary care to specific residents under the medical direction and supervision of the residents' attending physicians or the Medical Director of the facility with an emphasis on assessment, illness prevention and health care management including assistance in modifying the treatment regimen to meet the physical and psychosocial needs of the resident in accordance with established medical practices, requirements of the state and the policies and goals of the facility.
ESSENTIAL FUNCTIONS:
* Contribute to the facility's quality management including reduction of re-hospitalizations that were avoidable, development of safe discharge plans and culturally competent care.
* Assist the Quality Assurance and Performance Improvement (QAPI) Committee in developing and implementing appropriate plans of action to correct identified deficiencies.
* Assist in the development and implementation of facility policies and procedures as appropriate.
* Initiate requests for consultation or referral.
* Participate in facility surveys (inspections) made by authorized government agencies as necessary.
* Participate in the development of the annual facility assessment by assisting in identifying tools, training and resources required to effectively treat the resident population.
* Serve on the IDT and collaborate on conducting assessments and developing care plans for residents.
* Participate in resident discharge planning.
* Assess, diagnose and treat residents with acute, episodic or chronic illness independently or as part of a healthcare team and conduct a review every 30 days for each veteran assigned to the case load.
* Order, interpret and evaluate diagnostic tests to identify and assess resident's clinical problems and health care needs.
* Consult with the Medical Director and the attending physician regarding specific residents as necessary.
* Determine when to refer the resident to a physician or specialist for evaluation, supervision or directions.
* Collaborate with the interdisciplinary team (IDT), resident and family members on care planning and interventions that are individualized and person-centered; evaluate interventions and revise as appropriate.
* Provide preventive health care services.
* Conduct medication reviews in collaboration with the consultant pharmacist.
* Coordinate with physicians and the Medical Director for on-call rotation to provide appropriate coverage.
* Collaborate with the Clinic Competency Coordinator to schedule education and training required to provide proficient and culturally competent care including new adaptive equipment or treatments.
WORKING CONDITIONS/PHYSICAL REQUIREMENTS:
* Works in office areas as well as throughout the nursing services area (i.e. drug rooms, nurses' stations, resident rooms, etc.).
* Must be able to move intermittently throughout the workday.
* Is subject to frequent interruptions.
* Is involved with residents, personnel, visitors, government agencies/team members, etc., under all conditions and circumstances.
* Communicates with the medical staff, nursing personnel and other department directors.
* Works beyond normal working hours and on weekends and holidays when necessary. On-call availability up to 24 hours a day, 7 days per week or on a rotating schedule per facility requirements.
* Is subject to call back during emergency conditions (e.g. severe weather, evacuation, post-disaster, etc.).
* Attends and participates in continuing education programs.
* Is subject to injury from falls, burns from equipment, odors, etc., throughout the workday as well as to reactions from dust, disinfectants, tobacco smoke and other air contaminants.
* Is subject to exposure to infectious waste, diseases, conditions, etc., including TB and the AIDS and Hepatitis B viruses.
* May be subject to the handling of and exposure to hazardous chemicals.
* Must be able to see and hear or use prosthetics that will enable these senses to function adequately to ensure that the requirements of this position can be fully met.
* Must function independently and have flexibility, personal integrity and the ability to work effectively with residents, personnel and support agencies.
* Must meet the general health requirements set forth by the policies of this facility, which include a medical and physical assessment.
* Must be able to relate to and work with the ill, disabled, elderly, emotionally upset and, at times, difficult people within the facility.
* Must be able to push, pull, move and/or lift a minimum of 25 pounds to a minimum height of 5 feet and be able to push, pull, move and/or carry such weight a minimum distance of 50 feet.
* May be necessary to assist in the evacuation of residents during emergency situations.

* Must be able to relate information concerning a resident's condition.
* Must not pose a direct threat to the health or safety of other individuals in the workplace.
* Must be able to read, write, speak and understand the English language.
* Must demonstrate the knowledge and skills necessary to provide care appropriate to the age-related needs of the residents served.
REQUIRED EDUCATION AND EXPERIENCE:
* Must possess, as a minimum, a Master of Science in Nursing (MSN) from an accredited college or university.
* Prefer, as a minimum, three (3) year(s) experience as a Registered Nurse (RN).
* Must possess a current unencumbered active license to practice as an APRN in the state of Georgia.
* Must have the ability to obtain and maintain a DEA in the state of Georgia.
* Must be able to obtain and maintain an active American Heart Association BLS certification.
* Must be legally eligible to work in the United States.
NOTE:
As part of orientation, selected candidates will receive safety care training.
