Qualifications

Our mission-driven, member-focused company seeks an experienced and self-motivated Licensed Mental Health Therapist to serve our members' behavioral healthcare needs.

This role involves working within our value-based care model, emphasizing the right care at the right time, which aims to improve clinical outcomes and provide a sustainable workload for clinicians. The therapist will be integral to our Integrated Care Team, helping develop and implement tech-enabled psychotherapeutic care, integrating social and clinical care to support member wellbeing. The work will primarily be via telehealth, with some in-home and community visits as needed. You will collaborate with a multidisciplinary team including Psychiatric Nurse Practitioners and Community Guides, providing holistic, member-centered care for those with complex health and social needs.

Primary Responsibilities:

• Assess and diagnose behavioral health conditions, both chronic and acute.
• Formulate and adjust treatment plans in collaboration with the healthcare team.
• Incorporate behavioral health perspectives into comprehensive care plans.
• Conduct initial assessments, deliver therapeutic interventions, and connect members to community resources.
• Collaborate with other healthcare providers on treatment progress.
• Provide psychoeducation and promote mental health and disease prevention.
• Support referrals for additional care and advise on treatment plans.
• Build trusting relationships aligned with patient goals and values.
• Participate in case conferences for holistic care.
• Educate team members on behavioral health issues.
• Support discharge planning to prevent readmissions.
• Document care accurately in our EMR system.
• Contribute to clinical pathway development to enhance care quality.

Job responsibilities will evolve with business needs.

Experience and Skills:

• At least 3 years of outpatient experience with adults on Medicaid, Medicare, or dual-eligible.
• Experience in behavioral health care planning, management, and therapy.
• Experience with populations facing socioeconomic barriers.
• At least 1 year practicing telemedicine.
• Knowledge of trauma-informed, evidence-based psychotherapy.
• Comfort working alongside Community Guides in an integrated model.
• Proficiency with electronic medical records and technology.
• Strong clinical assessment skills.
• Ability to handle challenging situations calmly and fairly.
• Effective team collaboration skills.
• Proficiency in Microsoft Office, telecommunication, and video conferencing; G-Suite preferred.

Education & Qualifications:

• Active LCSW, LMFT, or LPCC license in Ohio.
• 18+ years old.
• Compliance with confidentiality and HIPAA regulations.
• Proficiency with EMR and related technology.
