Job summary

Springfield Memorial Hospital is seeking an Advance Practice Provider (Nurse Practitioner or Physician Assistant) for full-time, on-site work in the Wellness Center. The role includes managing weight loss programs (surgical and nonsurgical) and coordinating care under a physician’s direction.

Responsibilities emphasize collaboration with a multidisciplinary team, patient advocacy, teaching, and family counseling, with licensing and certifications as required by Illinois law.

Qualifications

• Certification as Nurse Practitioner and Illinois APN licensing.
• Minimum 1 year post-graduate APN experience.
• Valid CPR required; ACLS where required.
• Maintains DEA license where required.

Responsibilities

• Provide care coordination and evaluation for Wellness Center patients.
• Collaborate with Wellness Center multidisciplinary team and refer to other providers as needed.
• Teach, advocate for patients, and involve family counseling.
• Under physician supervision, coordinate the department or work area.
