Job summary

Ascend Medical is seeking an Advanced Practice Provider (NP or PA) to deliver relationship-centered care to seniors in premier metro Atlanta communities. This role offers flexible part‑time or full‑time options with strong support, comprehensive benefits, and a focus on geriatrics.

You will work with a dedicated clinical team, manage chronic conditions, and tailor plans to each patient’s goals in an on‑site senior living setting. Prior home health or hospice experience is valued.

Qualifications

• Master’s degree required.
• Minimum of 3 years of clinical experience, with strong preference for geriatric, hospice, or home health.
• Active DEA license (or eligibility to obtain).
