Job summary

Sandbar Health in the DC metro area is seeking an Advanced Practice Provider (NP/PA) to join a founding care team. Qualifications

  • 2+ years of experience in Family Medicine, Urgent Care, or related outpatient settings preferred

Responsibilities

  • Provide comprehensive primary care for adult and pediatric patients
  • Manage same-day, urgent, and low-acuity care across in-person, virtual, and asynchronous workflows

We are building a next-generation "payvider" — a physician-led concierge primary care practice and health plan in one — with an obsessive focus on the member experience, launching in the DC metro area.

Position summary

We are seeking an experienced Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to serve as a core member of a collaborative, patient-centered care team. APPs will play a critical role in delivering high-access, relationship-driven care through a blend of in-person, virtual, and asynchronous care modalities. APPs will work closely with physicians, behavioral health clinicians, health coaches, and care navigators within a POD-based care structure designed to maximize both patient experience and clinical outcomes.

This role is ideal for a clinician who thrives in fast-paced outpatient environments, values strong patient relationships, and is excited about helping build a modern healthcare delivery model from the ground up.

Core responsibilities
  • Provide comprehensive primary care services within scope of practice for adult and pediatric patients
  • Manage same-day, urgent, and low-acuity care needs through in-person, virtual, and asynchronous workflows
  • Support preventive care, chronic disease management, and proactive outreach initiatives
  • Collaborate closely with supervising physicians and interdisciplinary care team members

Clinical model

The Practice Utilizes a High-touch, Team-based POD Model Designed Around Smaller Patient Panels, Rapid Access, And Integrated Care Delivery. Each POD Supports Approximately 500–750 Patients And Includes Physicians, APPs, Health Coaches/medical Assistants, And Embedded Behavioral Health Support. The Care Model Emphasizes:

  • Same-day and next-day access
  • Concierge-level communication and responsiveness
  • Preventive and longitudinal care
  • Async-first workflows for low-acuity concerns
  • Technology-enabled operations to reduce administrative burden
  • Collaborative interdisciplinary care delivery
Preferred Qualifications
  • Licensed Nurse Practitioner (NP) or Physician Assistant (PA)
  • Active or eligible licensure in Washington, DC, Maryland, and/or Virginia
  • 2+ years of experience in Family Medicine, Urgent Care, Emergency Medicine, Concierge Medicine, or related outpatient settings preferred
  • Comfort managing both scheduled and same-day care needs
