Job summary

Sandbar Health is seeking a founding Medical Director to lead our physician-led concierge primary care with its own health plan in the DC metro area and Northern Virginia. You will be the first physician, seeing patients and shaping the care model while building the team and setting care standards.

In this full-time role, you will practice family medicine across the care journey, spend substantial time with patients, and help recruit clinicians as we grow pods, owning pathways and ensuring

Qualifications

• Board Certified in Family Medicine.
• Active or eligible medical licensure in DC, Maryland, and/or Virginia.
• Experience leading a founding care team and building a care model.

Responsibilities

• Practice full-scope outpatient Family Medicine for adults and children, in person and virtual.
• Lead the founding care team and set daily operations.
• Recruit and mentor clinicians as pods grow.
• Own clinical pathways and update them with evidence.
• Run preventive and chronic disease management to keep members healthy.
• Perform office-based procedures and women's health services.
• Collaborate with care navigation to coordinate patient care.

Sandbar Health is a physician-led concierge primary care practice with its own health plan attached. We deliver the care and we carry the cost of it, which means our incentives sit with our members' health instead of with the number of visits on a given day.

We are hiring our founding Medical Director. You would be the first physician in the practice, and will be instrumental in shaping how we deliver care to our patients. You will see patients, as well as shape the care model, help hire the team you work alongside, and set the standard for what good care looks like here.

What you would do

• Practice full-scope outpatient Family Medicine for adult and pediatric patients across the care journey, in person and virtually. We anticipate approximately 70% of your time will be spent on patient care
• Lead the founding care team (APPs, medical assistants, health coaches, and embedded behavioral health) and set how that team works day to day
• Help recruit and mentor the clinicians who come after you as we add pods
• Own our clinical pathways and standards, and revise them when the evidence or the reality on the ground says to
• Run preventive care and chronic disease management with an eye on keeping people healthy and out of the hospital
• Perform office-based procedures and women's health services within your scope
• Work closely with our care navigation team, so patients are not left to coordinate their own care

The practice

We built the practice around small panels and real access. Each pod is one physician, one medical assistant, and up to four APPs, with health coaching and behavioral health support built in. Panels run 500 to 750 patients per provider, to enable more time spent with each patient.

A Few Specifics Physicians Usually Want Up Front

• Panel: 500 to 750 patients per provider
• Pod staffing: 1 MA per physician, up to 4 APPs, plus health coaching and behavioral health
• EMR: Canvas, with ambient AI note-taking, so your charting does not follow you home as pajama time
• Access: same-day and next-day availability across in-person, virtual, and asynchronous visits
• After-hours and call: partner-led telemedicine platform

Because panels are small, visit volume runs lower than a standard primary care day. Appointments are longer, and low-acuity needs get handled asynchronously when that serves the member better than a trip to the office.

• Board Certified in Family Medicine
• Active or eligible medical licensure in DC, Maryland, and/or Virginia
• Comfortable caring for patients across the care journey, including preventive care, women's health, and common musculoskeletal complaints

Preferred

• Prior clinical leadership, whether as a medical director, lead physician, or similar
• A background in concierge, value-based, or lifestyle medicine. Urgent care, emergency medicine, and rural practice translate well too
• Someone who likes team-based care and is excited by the ambiguity of building something early
