Qualifications

• Active RN, and APRN/APRN-FPA/APNP or other APRN license(s) in state(s) of practice.
• Active national board certification in area of clinical practice.
• Active DEA registration prior to hire.
• Active Illinois Controlled Substance License prior to hire (if applicable).
• Active BLS and/or ACLS, PALS, NRP as required.

Responsibilities

• Assess patients and alter treatment plans based on clinical condition.
• Manage patient conditions with evidence-based care.
• Provide health promotion and disease management education.
• Collaborate with interdisciplinary healthcare teams.
• Document and code professional services accurately.
