Job summary

EPIC Health seeks an Advanced Practice Provider (PA or FNP) to join our Cicero, IL team. You will evaluate patients, diagnose and treat conditions, develop treatment plans, provide preventive care, and coordinate referrals in a fast-paced primary care setting.

The role offers a base salary of $110,000–$150,000 per year, plus quarterly bonuses up to $9,000 and the potential for a $36,000 annual bonus. Spanish language is preferred but not required.

Qualifications

• Active, unrestricted Illinois PA or FNP license.
• Graduation from an accredited PA or Nurse Practitioner program.
• Current national certification appropriate to your profession.
• Spanish language – preferred, not required.
• Strong clinical assessment, diagnostic, and treatment skills.
• Ability to provide primary care to patients across a range of ages.

Responsibilities

• Evaluate patients, obtain health histories, and perform examinations.
• Diagnose and treat acute and chronic conditions within scope.
• Develop treatment plans and provide follow-up care.
• Provide preventive care, health education, and counseling.
• Order, perform, and interpret diagnostic testing including labs and EKGs.
• Prescribe medications and therapies per laws and policies.
• Perform office-based procedures including injections, wound care, suturing, and immunizations.
• Collaborate with physicians and clinical team to provide care.
• Coordinate referrals and follow-up testing.
• Work with staff to support efficient patient care.
• Communicate effectively with patients, families, providers, and staff.

Compensation

• Base Salary:$110,000–$150,000 annually, based on experience

What You'll Do

• Evaluate patients, obtain comprehensive health histories, and perform physical examinations.
• Diagnose and treat acute and chronic medical conditions within your scope of practice.
• Develop individualized treatment plans and provide appropriate follow-up care.
• Provide preventive care, health education, and counseling to patients and families.
• Order, perform, and interpret appropriate diagnostic testing, including laboratory studies, EKGs, and other diagnostics within scope.
• Prescribe medications and therapies in accordance with applicable laws, regulations, and organizational policies.
• Perform appropriate office-based procedures, including injections, wound care, suturing, and immunizations.
• Collaborate with physicians and other members of the clinical team to provide comprehensive patient care.
• Coordinate referrals, diagnostic testing, follow-up care, and other services as needed.
• Work closely with medical assistants, nurses, technicians, and other clinical staff to support efficient patient care.
• Communicate effectively with patients, families, providers, and staff.

Clinical Quality & Documentation

• Maintain accurate, timely, and compliant medical records.
• Follow clinical protocols, evidence-based practices, and organizational policies.
• Promote patient safety, quality care, and a positive patient experience.
• Participate in quality improvement initiatives and team meetings.
• Maintain appropriate professional licensure, certifications, and clinical credentials.

Required Qualifications

• Active, unrestricted Illinois Physician Assistant (PA) or Family Nurse Practitioner (FNP) license.
• Graduation from an accredited PA or Nurse Practitioner program.
• Current national certification appropriate to your profession.
• Fluency in Spanish – preferred, not required.
• Strong clinical assessment, diagnostic, and treatment skills.
• Ability to provide primary care to patients across a range of ages.
• Excellent communication and interpersonal skills.
• Ability to work effectively in a fast-paced, patient-centered environment.
• Strong commitment to quality, patient safety, and compassionate care.

Preferred Qualifications

• 2–5 years of experience in primary care and/or pediatrics preferred, open to new graduates.
• Experience working with culturally diverse patient populations.
• Experience in a community health center, multi-specialty practice, or other outpatient setting.
• Experience with common outpatient procedures.
• Strong interest in preventive care and long-term patient relationships.
