Endeavor Health in Elmhurst, IL is seeking an Advanced Practice Provider (APN/PA) to join the Family Medicine Department. You will provide comprehensive outpatient care under the supervision of collaborating physicians, performing exams, ordering tests, and managing treatment plans.

The role supports flexible, family-friendly hours with a 40-hour work week and rotating outpatient call. Candidates must have APN/PA licensure in IL and BLS/CPR certification; DEA license is required by law for

• Experience in outpatient primary care or community health settings preferred.
• Ability to work autonomously within an interdisciplinary team.
• Fluent or strong English communication skills preferred.

Position Highlights

Hourly Pay Range: $56.44 - $70.58 - The hourly pay rate offered is determined by a candidate's expertise and years of experience, among other factors.

Job Description

Exciting opportunity to join Endeavor Health in the Family Medicine Department. Under the direction of the collaborating physician(s), provides care to patients. Obtains detailed history and performs physical examinations, orders imaging, lab tests and diagnostic procedures and interprets results. Instructs patients on the prescribed therapeutic regimen. Maintains records and writes progress notes on status, treatments and procedures. Collaborates with physicians and the health care team to achieve optimal patient outcomes. Prescribes pharmacologic and non-pharmacologic therapies as appropriate. Demonstrates evidence-based practice. Participates in the development, implementation, and evaluation of standards of practice in the specialty. Assesses the learning needs of patients and families across the continuum. Employs strategies, interventions and teaching to promote, maintain, and improve health. Provides comprehensive health assessments, formulates differential diagnoses, and prescribes treatments under the protocol in the direct management of acute and chronic illness and disease. Promotes wellness and preventative health in the outpatient setting. Works autonomously and in interdisciplinary teams as resources and consultants. Positively contributes to safety outcomes and promoting high-quality patient experience. Participates in the development, implementation, and evaluation of service line goals. Supports the strategic plan of the organization. Adheres to all Endeavor Health policies and procedures. This advanced practice provider will see new pts, established pts, walk ins, etc.

Qualifications

License: Advanced Practice Licensure (APN) or Physician Assistant (PA) Licensure in the State of IL required; Federal DEA and Illinois Controlled Substance License required

Education: Completion of master's level program in specialty area preferred

Certification: BLS/CPR certification required
