Job summary

Telos Health Systems seeks an experienced Advanced Practice Provider (NP/PA) to serve as the in-house SNF-ist in Asheville, NC. You’ll deliver general medicine care to geriatric residents across SNFs and ALFs, with autonomy and collaborative support from a care team.

This W-2 role offers pay-per-encounter remuneration and a sign-on bonus up to $15,000, plus a hybrid schedule and generous benefits designed to support professional growth and resident well-being.

Qualifications

• Active North Carolina APRN/APN or PA-C license.
• Current DEA registration.
• Master’s degree from accredited NP/PA program.
• Strong communication and leadership skills.
• Ability to work effectively in a fast-paced environment.

Responsibilities

• Serve as facility’s primary internal medicine provider (SNF-ist).
• Manage care for short-term rehab and long-term residents.
• Perform comprehensive health assessments, physical exams, and diagnoses.
• Manage acute and chronic conditions following established protocols.
• Prescribe medications and treatments as appropriate.
• Conduct monthly stability visits for long-term residents.
• Evaluate residents with changes in condition.
• Participate in Advanced Care Planning.
• Collaborate with interdisciplinary teams, medical directors, and attending physicians.
• Complete accurate documentation and CPT coding.
• Facilitate referrals to specialists and other healthcare providers.

Position Overview

Telos Health Systems is seeking an experienced Advanced Practice Provider — either a Nurse Practitioner (NP) or Physician Assistant (PA) — to serve as an in-house SNF-ist, providing general medicine care to geriatric residents in Skilled Nursing Facilities (SNFs) and Assisted Living Facilities (ALFs) in the Asheville, NC area.

What You’ll NeedRequired

• Active, unencumbered Nurse Practitioner (APRN/APN) or Physician Assistant (PA-C) license in North Carolina
• Current DEA registration
• Master’s degree from an accredited Nurse Practitioner or Physician Assistant program
• Strong communication and clinical leadership skills
• Ability to work effectively in a fast-paced environment

Preferred

• At least 1 year of experience in a Skilled Nursing Facility (SNF) or long-term care setting
• Experience caring for geriatric populations
