Job summary
Telos Health Systems is seeking an experienced Psychiatric Mental Health Nurse Practitioner (PMHNP) for SNF-ists to provide geriatric care in Skilled Nursing and/or Assisted Living settings. The role offers a 2-day-per-week daytime schedule and potential full-time expansion when paired with nearby facilities.
The PMHNP will lead internal medicine services, collaborate with the interdisciplinary team, perform comprehensive exams, and manage patient care under established protocols.
Qualifications
- Certification as a PMHNP.
- Active license and DEA number.
- Experience in nursing or medical fields.
- Minimum one-year experience in a Skilled Nursing setting (preferred).
Responsibilities
- Lead internal medicine services as the SNF-ist and coordinate with the interdisciplinary team.
- Oversee day-to-day care for short- and long-term residents.
- Provide comprehensive health assessments and prescribe treatments per protocol.
- Prescribe medications and manage treatments.
- Perform comprehensive physical examinations.
- Develop and implement patient management policies and procedures.
- Facilitate referrals to other healthcare professionals and medical facilities.
- Communicate with attending physicians and specialists regarding patient care.
- Be comfortable with documentation and CPT coding.
- Monthly stability visits for long-term residents.
- Evaluations for patients with changes in condition.
- Advanced care planning.

Telos Health SystemsstaffsPsychiatric Mental Health Nurse Practitioners, PMHNPs and General Medicine Nurse Practitioners, to improve residents’ quality of life and overall well-being while simultaneously reducing return to hospital stays.
- As the SNF-ist, you will act as a Skilled Nursing Facility’s leading provider of internal medicine services. You will collaborate with the facility’s interdisciplinary team and physicians, serving as a day-to-day clinical leader while helping oversee the care of all the facility’s residents.
- You will work with the entire facility’s patient population - short and long-term residents - overseeing their day-to-day care with the assistance of the facility’s nursing staff and guidance from the facility’s medical directors and attending physicians.
- Provide comprehensive health assessments, differential diagnosis, and prescribe treatments under the protocol in the direct management of acute and chronic illness and disease.
- Prescribe all necessary medications and treatments.
- Perform comprehensive physical examinations of patients.
- Develop and implement patient management policies and procedures.
- Facilitate referrals to other healthcare professionals and medical facilities.
- Communicate with attending physicians and/or specialists regarding patient care.
- Be comfortable and proficient with documentation and CPT coding.
- Monthly stability visits for long term residents
- Evaluations for patients exhibiting changes in condition.
- Advanced Care Planning
- Certification as a Psychiatric Mental Health Nurse Practitioner (PMHNP)
- Must have an active license and DEA #
- Previous experience in nursing or other medical fields
- Minimum one-year experience in a Skilled Nursing setting (preferred)
- Ability to build rapport with patients and their loved ones.
- Ability to thrive in a fast-paced environment.
- Excellent written and verbal communication skills
- Strong leadership experience with emphasis on teamwork in a healthcare setting
