Job summary

Titan Placement Group is hiring an Advanced Practice Provider (NP or PA) to float across Jacksonville-area outpatient adult primary care clinics. You will handle acute and chronic conditions, preventive care, and referrals with support from a multidisciplinary team.

The role offers a Monday–Friday schedule (8:00 AM–5:00 PM), travel between sites, mentorship from physicians, and a collaborative, value-based care model in a stable, physician-led organization.

Qualifications

• Board-certified NP or licensed PA with at least 2 years of adult primary care experience.
• Able to independently manage acute and chronic conditions.
• Medicare Advantage experience preferred; Spanish proficiency is a plus.

Responsibilities

• Provide outpatient primary care for adults across multiple Jacksonville-area clinics.
• Manage acute and chronic conditions, preventive care, wellness visits, medications, diagnostics, and referrals.
• Collaborate with a multidisciplinary care team in a physician-led value-based care model.

We’re hiring an Advanced Practice Provider (NP or PA) - Float who wants a predictable weekday schedule, variety in their workday, and the stability of an established physician-led primary care organization.

You’ll step into an outpatient role focused exclusively on adult primary care, providing coverage across established Jacksonville-area clinics. You’ll manage acute and chronic conditions, preventive care, wellness visits, medications, diagnostics, and referrals with support from a multidisciplinary care team.

Compensation & Benefits
