St. Joseph’s/Candler Health System in Savannah, GA is seeking an Advanced Practice Provider in Endocrinology to obtain histories, perform exams, and manage patients across complex endocrine conditions under supervision.
The APRN/PA will diagnose diabetes, thyroid, adrenal, and pituitary disorders; develop personalized treatment plans; order tests and CGM, and collaborate with endocrinologists, diabetes educators, dietitians, and primary care to ensure coordinated care across the health system.
The Advanced Practice Provider, Endocrinology obtains a complete history and examination on new patients or established patients; possesses knowledge of the spectrum of endocrine diseases, recognizes presentations of endocrine diseases in laboratory studies, and other diagnostic tests; may educate, instruct, and counsel patients and their families regarding their health condition and directs them to appropriate available resources. Documents patient encounters appropriately; identifies patients presenting with complex endocrine conditions and confers with supervising physician appropriately; facilitates referrals to other providers, medical facilities, or health/social service agencies when appropriate.
Master's in Nursing APRN or Master's in Physician Assistant Studies - Required
Graduation from accredited Nurse Practitioner or Physician Assistant program - Required
1 Year as a physician extender - Preferred
5 Years clinical Endocrinology or Internal Medicine experience, in a licensed capacity - Required
Professional APRN or PA License in State of Practice - Required
Basic Life Support (BLS) - Required
CDE - Preferred within 24 months of hire
Conduct detailed patient evaluations, including medical histories, physical examinations, and risk assessments; diagnose acute and chronic endocrine conditions such as diabetes, thyroid disorders, adrenal and pituitary diseases, osteoporosis, and metabolic disorders.
Formulate and implement individualized, evidence-based treatment plans, including pharmacologic and non-pharmacologic interventions; monitor patient response and adjust therapies as needed to achieve optimal outcomes.
Order, perform, and interpret laboratory tests, imaging studies, CGM management and interpretation, insulin pump management, and other diagnostic procedures relevant to endocrine care; integrate findings into clinical decision-making.
Collaborate with endocrinologists, primary care providers, diabetes educators, dietitians, and other healthcare professionals to ensure coordinated, continuous care across the health system. Refer to specialists as needed for optimizing patient outcomes. Readily co-manage Endocrine patients with Endocrinologists.
Provide education to patients and families regarding disease processes, treatment plans, medication management, and lifestyle modifications; promote patient engagement and self-management, particularly for chronic conditions. Educate on diabetic site, nutrition, and management.
Participate in quality improvement initiatives, utilize population health tools to identify care gaps, and support adherence to clinical guidelines and performance metrics within the integrated health system.
Maintain accurate, timely, and complete documentation in the electronic health record (EHR); ensure compliance with organizational policies, billing and coding standards, and applicable regulatory requirements.
