Endeavor Health is seeking an Advanced Practice Provider (APN/PA) for Immediate Care at Deerfield or Skokie offices. This part-time role totals 36 hours per week with shifts Monday–Friday 8:00 AM–8:00 PM and weekend/holiday rotation 8:00 AM–4:00 PM.

The outpatient setting emphasizes prompt, coordinated care under collaborating physicians. The APN/PA will perform histories and exams, order tests, interpret results, and develop treatments and patient education plans.

Qualifications

• Requires Advanced Practice Nurse Licensure (APN) or PA licensure in IL
• Federal DEA and Illinois Controlled Substance License required
• MSN/MS or NCCPA certification; FNP for APRNs preferred

Responsibilities

• Provide care to patients under physicians' direction
• Take detailed histories and perform physical exams
• Order imaging, labs and diagnostic tests; interpret results
• Prescribe pharmacologic and non-pharmacologic therapies and develop care plans
• Maintain records and write progress notes; follow patients through treatment

Job description

Exciting opportunity to join Endeavor Health Immediate Care! Under the direction of collaborating physicians, provides care to patients. Obtains detailed history and performs physical examinations, orders imaging, lab tests and diagnostic procedures and interprets results. Instructs patients on the prescribed therapeutic regimen. Maintains records and writes progress notes on status, treatments, and procedures. Follows patients through their course of treatment in the immediate care. Collaborates with physicians and the health care team to achieve optimal patient outcomes. Prescribes pharmacologic and non-pharmacologic therapies as appropriate. Demonstrates evidence-based practice. Participates in the development, implementation, and evaluation of standards of practice in the specialty. Assesses the learning needs of patients and families across the continuum. Employs strategies, interventions and teaching to promote, maintain, and improve health. Works autonomously and in interdisciplinary teams. Positively contributes to safety outcomes and promotes high quality patient experiences. Participates in the development, implementation, and evaluation of service line goals. Supports the strategic plan of the organization. Adheres to all Endeavor Health policies and procedures.

Qualifications

• License: Advanced Practice Nurse Licensure (APN) or Physician Assistant (PA) licensure in the State of IL required; Federal DEA and Illinois Controlled Substance License required


• Education: Completion of Master’s level program in specialty area (MSN/MS or NCCPA certified PA) FNP certification required for APRNs


• Certification: BLS/CPR for Healthcare Provider certification required (in-person skills validation a must)


• Experience: Minimum 2 years Emergency Department or IC experience as an Advanced Practice Nurse or PA required to be considered for the position.
