Job summary

Commonwealth Pain & Spine seeks an Advanced Practice Provider (APRN/PA) to join our pain management team in Mount Carmel, IL. You will evaluate patients, develop care plans, and perform injections in collaboration with physicians.

The role requires an active APRN license, DEA registration preferred, and the ability to obtain clinical privileges in Indiana. We value precise diagnostics, compassionate care, and teamwork in a multimodal treatment approach.

Qualifications

• Active, valid APRN certificate.
• DEA Registration preferred.
• Must hold a valid, unrestricted Indiana license and be able to obtain clinical privileges to practice medicine.

Responsibilities

• Identify and assess patient’s clinical issues and health care needs.
• Record physical findings and formulate plan and prognosis.
• Evaluations and re-evaluations of established patients.
• Refer patients to physicians for consultation or specialized resources.
• Discuss cases with physician to prepare comprehensive care plan.
• Perform pain management injections per practice protocols.
• Prescribe/order diagnostics, therapeutics, medications, and pain management procedures.
• Work independently as well as part of a team with flexibility and initiative.
• Support the organization’s vision and participate in performance improvement.
• All other duties as assigned.

Advanced Practice Provider (APP) - APRN/PA

Duties And Responsibilities

• Identify and assess patient’s clinical issues and health care needs
• Record physical findings, and formulates plan and prognosis, based on patient’s condition and the doctor’s plan of treatment
• Evaluations and Re-evaluations of established patients
• Refers patients to Physician for consultation or to specialized health resources for treatment when needed.
• Discusses the case with Physician and other health professionals to prepare a comprehensive patient care plan
• Performs pain management injections for specific diagnoses according to practice protocols
• Prescribe/Order as medically indicated:- Diagnostics- Therapeutics- Medication- Pain Management Procedures- DME
• Eagerness and ability to work independently as well as part of a team with flexibility and willingness to learn and take initiative on variety of tasks and projects
• Supports the vision and culture of the organization. Demonstrates personal commitment through active involvement in the performance improvement process
• All other duties as assigned

Skills And Abilities

• Highly proficient with computers especially: EMR Software, Meaningful Use, Microsoft Word, Excel, Outlook
• Ability to manage high levels of stress and capable of making sound difficult decisions
• Very structured, organized, consistent, pleasant, cheerful, yet professional personality, who will earn the respect of patients and co-workers immediately
• Must be incredibly punctual. Great attendance, required

Qualifications, Education, And Experience (APRN)

• Active, valid (APRN) Nurse Practitioner certificate, required
• DEA Registration, preferred
• Qualifications, Education, and Experience (PA-C):
• Must have a valid, unrestricted professional license in state of Indiana and be able to obtain clinical privileges to practice medicine

Physical Requirements

The physical demands of this position will include sitting and standing with occasional light to medium lifting. Ability to help lift patients and transport in wheelchair from clinic to parking lot.
