Position Details

We are searching for experienced, dynamic advanced practice providers to join one of our primary care sites. As part of a primary care medical home, we serve 28,000 patients annually across six health care centers, a 34‑chair dental clinic, and a mobile health center. Our multidisciplinary teams provide outpatient services focused on medical, emotional, and social care for adults and children. The teams include physicians, family nurse practitioners, physician associates, integrated behavioral health professionals, nurses, medical assistants, registration staff, and team leads. All sites are outpatient only – no inpatient or obstetrical responsibilities.

Position Highlights

• Full‑time employees generally work a four‑day week, with four weeks of paid vacation per year to start, in addition to a week of CME.
• Participation in the on‑call rotation is optional, with additional compensation.
• We are a National Health Service Corp (NHSC) loan repayment site, with a medical HPSA score of 19.
