Job summary

inFORME healthcare, LLC in Madison, WI, seeks a dynamic Advanced Practice Provider (NP or PA) to manage higher-acuity inpatients in a post-acute hospital. You will care for patients with trach/vent/complex wounds and collaborate with hospitalists on a 7 on 7 off schedule, including weekend rotations.

This role requires 3–5 years of acute inpatient experience and Wisconsin licensure, with autonomous practice.

Qualifications

Responsibilities

• Manage acutely ill inpatients with high diagnostic complexity, including tracheostomy/ventilator cases and complex wounds.
• Collaborate with hospitalists and clinical leadership to maintain quality metrics and outcomes.
• Participate in weekend rotations and 7-on/7-off shift schedule.
• Contribute to implementing proprietary protocols and program components.

inFORME healthcare is looking for a dynamic, clinically adept Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to work with our Physician leaders in a post-acute hospital. Your role will be to manage inpatients that have a higher acuity in the Madison, WI area.
The patient diagnostic complexity involves managing trach/vent/complicated wound and multiple co-morbidities. You must be comfortable managing acute changes in place.
This position would involve working a shift schedule of 7 on 7 off. This will include weekend rotations to manage patients and collaborate with our Hospitalists. Additionally, you would collaborate with corporate leadership to assist in maintaining optimal momentum of our stellar quality metrics, program components and implementing our proprietary protocols to achieve clinical outcomes.
