Job summary

Platte River Medical Clinic is seeking an Advanced Practice Provider to deliver acute and episodic care within a Family Practice setting. You will evaluate, treat, and manage care for a diverse patient population, collaborate with a supportive team, and contribute to outcomes in a mission-driven clinic.

Ideal candidates have PA or NP certification, Colorado licensure, and 2+ years in Family/Internal Medicine or Urgent Care.

Qualifications

• Successful completion of Physician Assistant or Nurse Practitioner Master’s Program.
• Preferred +2 years in Family Medicine, Internal Medicine or Urgent Care.

Responsibilities

• Provides medical evaluation, treatment and consultation services to patients.
• Responds to emergencies and answers medical questions.
• Obtains and documents history, review of systems, examination, medications and allergies, assessment and plan for each patient.
• Provides medical orders for prescriptions, refills, and referrals.
• Performs routine medical procedures as needed.
• Provides referrals to medical specialists.
• Reviews test results and notifies patients.
• Provides patient education as needed.
• Maintains confidentiality and responds to patient questions.
• Completes documentation and paperwork timely.

In this role, you will play a crucial role in delivering acute and episodic care to a diverse patient population in a Family Practice setting. Duties and Responsibilities include the following

• Provides medical evaluation, treatment and consultation services to patients.*
• Responds to emergencies and answers medical questions.*
• Obtains and documents appropriate chief complaints, past medical, family, and social history, review of systems, examination, medications and allergies, assessment, and plan for each patient.*
• Provides medical orders for prescription refills, administration of medications, ancillary testing, and/or referrals.*
• Performs routine medical procedures that may include suturing/laceration repair, splinting, foreign object removal, etc.*
• Provide referrals to medical specialists.*
• Reviews ancillary test results and coordinates notification to patients.*
• Provides patient education as needed.*
• Maintains availability to patients personally or through clinical staff to answer questions and relay information regarding their care.*
• Completes appropriate paperwork to comply with insurance and reimbursement guidelines.*
• Completes all documentation and paperwork in a timely and accurate manner.*
• Manages personal clinical inbox.*
• Attends provider meetings as requested.*
• Maintains patient confidentiality at all times.*

Education and Experience

Successful completion of Physician Assistant or Nurse Practitioner Master’s Program.

Preferred Experience +2 years in Family Medicine, Internal Medicine or Urgent Care

Certificates and Licenses

• State of Colorado Medical License
• DEA certificate
• BLS/CPR certificate
• ACLS/PALS certification
