Job summary

Millennium-Physician-Group in Bradenton, FL is seeking a Nurse Practitioner for general/primary care in an ambulatory setting. You will independently manage acute and chronic conditions under physician collaboration, delivering evidence-based, patient-centered care.

Responsibilities include evaluating patients, developing treatment plans, immunizations, preventive care, health education, and coordination with a multidisciplinary team to improve outcomes and access to care.

Qualifications

• Master's-level nursing education and NP licensure.
• Ability to diagnose and manage acute and chronic primary care conditions.
• Experience with preventive care, immunizations, and health maintenance.
• Collaborates with physicians and care teams in a value-based care model.

Responsibilities

• Evaluate, diagnose, and manage acute and chronic primary care conditions.
• Develop individualized treatment plans including medications and preventive services.
• Order and interpret labs, imaging, EKGs, and point-of-care tests.
• Perform office-based procedures such as minor wound care and injections.
• Conduct annual wellness visits and transitional care management.
• Educate patients and caregivers on diagnoses and self-management.
• Document encounters in the EHR and support quality initiatives.
