Job summary

Cabarrus Rowan Community Health Centers is seeking an Advanced Practice Provider (APP) to support care delivery under physician supervision. You will perform histories, physicals, order tests, administer minor procedures, prescribe medications, and educate patients across assigned clinic locations.

The ideal candidate holds a master's degree as a Physician Assistant or Nurse Practitioner and current NC APP licensure with DEA certification.

Qualifications

• Proficient with Microsoft Office and email communications.
• Clear, concise communication with patients and staff.
• Demonstrated leadership, teaching and evaluation skills.
• Willingness to travel to all CRCHC locations for coverage.

Responsibilities

• Engage in patient-centered care and family-inclusive services.
• Provide medical evaluation, treatment and consultation; respond to emergencies.
• Document history, exam findings, assessment and plan; educate patients.
• Order refills, administer medications, and arrange referrals as needed.
• Review test results and coordinate follow-up with patients.
• Assist supervising physician with care across settings (home health, nursing homes, hospice).
• Be available to patients through staff to answer questions.
• Complete appropriate paperwork to meet insurance and compliance requirements.
• Educate patients and families on disease management and care plans.
• Train supervise and supervise medical students and APP learners as needed.
• Participate in quality improvement and program monitoring.
• Review peer charts and provide feedback to support evidence-based care.
• Comply with incentives, regulatory and certification reporting requirements.
• Direct the care activities of MA and nursing staff as required.
• Perform other duties as assigned by physician and leadership.
