Job summary

The Advanced Practice Clinician (NP/PA) is service/physician aligned, managing a distinct patient population. The APC may be required to perform some outpatient and inpatient care and facilitate discharges/outpatient follow up.

The APC has a strong foundation in evidenced based practice/education and is responsible for ensuring adherence to clinical pathways/protocols and ensures an environment of safe quality care.

Qualifications

• Licensed NP or PA in Tennessee.
• Strong documentation and communication skills.
• Experience with inpatient/outpatient care and discharge planning.

Responsibilities

• Assess health status and perform H&P for acute and complex patients.
• Order and interpret diagnostic tests and manage care plans.
• Coordinate daily schedules with the attending team and document progress.
• Provide patient education, and participate in discharge planning and follow-up.
