Job summary

Vanderbilt University Medical Center is seeking an Advanced Practice Clinician (NP/PA) to provide service-aligned care for a distinct patient population in a high-acuity environment. The APC brings evidenced-based practice, professional judgment, and strong collaboration with the physician team to ensure safe, quality care and adherence to clinical pathways.

The role may involve outpatient and inpatient care, discharge planning, and ongoing professional development.

Qualifications

• APC with NP/PA credential and Tennessee licensure.
• Experience in inpatient/outpatient care and discharge planning.
• Ability to document thoroughly and follow clinical pathways.

Responsibilities

• Assess health status and performs history and physical exams for acute and complex patients.
• Orders and interprets diagnostic tests and documents results.
• Coordinates daily clinic schedule with the attending physician team and follows plan of care.
• Documents progress notes, transfer/discharge summaries, and clinic notes.
• Provides patient and family education and supports continuity of care.
