Job summary

Eventus WholeHealth seeks an Advanced Practice Clinician (NP or PA) to deliver comprehensive primary care in skilled nursing and senior living settings. You will round, manage chronic conditions, and coordinate care with interdisciplinary teams, partnering with facility staff to drive preventive care and improved outcomes.

Strong collaboration skills, compassionate communication, and licensure in the practicing state are required; experience in geriatrics or internal medicine is preferred.

Qualifications

Current NP or PA licensure in the state of practice.
Experience in geriatrics, family medicine, or internal medicine preferred.
Exceptional patient-centered service skills and collaborative work with residents, families, and staff.

Responsibilities

Provide comprehensive primary care within skilled nursing and senior living communities.
Perform routine rounding and proactive chronic disease management.
Coordinate care across interdisciplinary teams and facilities partners.
