Eventus WholeHealth in North Carolina is seeking an Advanced Practice Clinician (NP or PA) to provide comprehensive primary care in skilled nursing and senior living communities. You will perform routine rounds, manage chronic diseases, and coordinate care with interdisciplinary teams and facility partners.

This role emphasizes proactive, value-based care, patient- and family-centered communication, and a collaborative approach.

Qualifications

• Current NP or PA licensure in the state of practice.
• Professional patient care experience.
• Experience in geriatrics, family medicine, or internal medicine preferred.
• Exceptional patient-centered service skills, including compassionate communication, active listening, professionalism, and the ability to build trusting relationships with residents, families, and facility staff
• Strong collaboration skills with the ability to work effectively across interdisciplinary teams

Responsibilities

• Provide comprehensive primary care services within skilled nursing and senior living communities.
• Perform routine rounding and proactive chronic disease management.
