Eventus WholeHealth is seeking an Advanced Practice Clinician (NP/PA) to deliver comprehensive primary care within skilled nursing and senior living communities. You will round, manage chronic conditions, coordinate care with interdisciplinary teams, and collaborate closely with facility partners to emphasize preventive care and better resident outcomes.

Qualifications

• Current NP or PA licensure in the state of practice.
• Previous professional patient care experience.
• Experience in geriatrics, family medicine, or internal medicine preferred.
• Exceptional patient-centered service skills, including compassionate communication, active listening, professionalism, and the ability to build trusting relationships with residents, families, and facility staff
• Strong collaboration skills with the ability to work effectively across interdisciplinary teams

Responsibilities

• Provide comprehensive primary care within skilled nursing and senior living communities.
• Perform routine rounds and proactive chronic disease management.
