Alignment Health is pioneering home-based senior care with a CAW program that integrates in-home assessments, telehealth, and a collaborative care team. This Advanced Practice Clinician role emphasizes proactive, compassionate care for vulnerable members with a balanced hybrid schedule.

You will work two days in the field and three days remotely, building care plans with an interdisciplinary team to reduce gaps in care and improve outcomes. Strong credentials and licensing are essential.

Formación

• Master's degree from an accredited NP or PA program.
• Active California NP or PA license.
• Active NP or PA board certification.
• Current NPI number, DEA registration, and BLS certification.
• Valid driver's license and automobile insurance.

Responsabilidades

• Conduct in-home assessments and health screenings for Alignment members two days per week.
• Provide telehealth support remotely from home three days per week.
• Educate patients and caregivers by aligning care plans with health goals.
• Identify diagnoses suitable for care management and active medical management.
• Develop individualized care plans with the Regional Medical Officer and interdisciplinary team.
• Coordinate care across multiple providers including primary care and specialists.
