Job summary

NHS Ayrshire & Arran is recruiting Advanced Nurse Practitioners to lead and deliver high-level nursing care in the community setting, supporting prevention and early discharge goals. This development post offers opportunities for progression and enhanced independent practice.

Position

In line with the National Discharge Without Delay and Home First approach, Community Nursing is key to South Ayrshire HSCP’s approach to prevention, supporting and managing individuals in their own home with a focus of reducing demand on primary, acute care and step up and step down models. South Ayrshire HSCP are looking to recruit Advanced Nurse Practitioners / Trainee Advanced Nurse Practitioner.

This post provides a leadership role to a growing community nursing staff group.

Hours

3x posts - 36 hours per week, with potential across 7 day working. This post is offered on a permanent basis.

UK Driving Licence

A UK driving licence is essential for this role. Please note, for all vacancies where a driving licence is required this must be a full UK/EU/EEA licence.

What You Will Do

Relevant post registration experience, within a community/hospital setting. Post holder will be a Registered Nurse at Masters level in advanced practice and are assessed as competent in this level of practice with current NMC registration (or willing to work towards this level of practice). As a clinical leader they have the freedom and authority to act and accept the responsibility and accountability for those actions. This level of practice is characterised by high level autonomous decision making, including assessment, diagnosis, treatment including prescribing, of patients with complex multi-dimensional problems. Decisions are made using high level expert, knowledge and skills. This includes the authority to refer, admit and discharge within appropriate clinical areas.

Providing expert clinical care and management of both the stable and acutely unwell person(s), making complex, autonomous decisions using advanced knowledge and skills, having overall responsibility for the assessment of care needs and the delivery of advanced nursing care to personal caseload.

Working collaboratively with community disciplines, as well as care home care and clinical staff, the multidisciplinary team within the locality GP Practice(s), as part of the Health and Social Care partnership, to prevent admissions to hospital, promote early discharge from acute/community hospital settings; to respond to existing or potential ill health issues in the person’s home, and to contribute to a reduction in unplanned acute hospital admissions.

Knowledge, Training And/or Experience Required For This Post

Treatment of common conditions with GP/Nurse Consultant support, discussion or review of complex presentations as needed.

Performing clinical assessment, initiate investigations, determine differential diagnosis and undertake appropriate first line interventions for the unwell patient (supported by a structured competency-based framework).

Utilising expert knowledge base and clinical competence, in partnership with all members of the multi-disciplinary team, ensuring a high standard of clinical care, in accordance with the Nursing & Midwifery Council (NMC) and Health & Safety legislation.

Providing leadership, clinical support and advice to nursing and allied health professionals, making appropriate referrals to and co-ordinating the contribution of other members of the multi-disciplinary team as well as care home care and clinical staff. Including close working and development alongside District Nurse Specialist Practitioner’s.

Participating in the development and provision of education and training to nursing and allied health professionals within NHS Ayrshire & Arran.

Facilitating and participating in quality improvement projects, quality assurance programmes and research, promoting evidence-based practice. This is a development post therefore there will be data gathering elements to the role focusing on prevention of hospital admission and patients care being provided in the least dependant setting for their needs.

A Post Graduate Diploma in Advanced Clinical Practice, including a nurse independent prescribing qualification is essential. In the absence of this the successful candidate will be supported to undertake academic study towards the above post graduate qualifications. Applicants who partially meet the essential criteria will be considered for a trainee Band 7 (AfC Annex 21 applies) and where required, you will be provided with a programme of training during your initial period of employment, specifically around managing the acutely unwell adult, physical health assessment, advanced/immediate life support and area specific training.
