Qualifications

• Provides ADL support including hygiene, mobility and meals.
• Collects vital signs and daily weights.
• Assists with basic nursing duties under RN supervision.
• Documents care and coordinates with care team to ensure patient safety.

Responsibilities

• Be the primary facilitator of ADLs: hygiene, toileting, mobility, meals.
• Collect vital signs & daily weights and perform point of care testing.
• Turn and reposition patients; assist with dressing and Foley/IV removal.
• Document in medical records and communicate changes to licensed staff.
• Coordinate patient transport and ensure equipment checks are performed.
