Job summary

HarmonyCares offers an in-home geriatric APP role within a growing primary care team. Monday to Friday, no weekends or evenings, focusing on chronic conditions in a high-risk patient panel in a value-based setting.

We support NP/PA clinicians with leave-free schedule, comprehensive benefits, and resources like CME, UpToDate, tail malpractice coverage, and a company car in select markets.

Qualifications

• Licensed as NP or PA in the state where HarmonyCares Medical Group office located.
• Federal DEA license/eligibility.
• Geriatric training/experience preferred.
• Active CPR/BLS certification.
• Board certification.
• Must maintain a valid driver’s license and good driving record.

Responsibilities

The Advance Practice Providers (NP/PA) we seek are those who have directly related experience in home health, geriatrics, and primary care. The Nurse Practitioner who wants to practice medicine with a focus on patient care, not volume. We want our clinicians to take the time needed to truly address the patient’s needs.
