Job summary

Sheppard Pratt is seeking a Case Management - Unlicensed professional for a full-time, on-site role in Maryland. Qualifications

• High school diploma or GED with 1 year of related experience.
• Bachelor’s degree preferred.
• Driver’s license with 3 points or less and access to insured vehicle.

Responsibilities

• Assess and monitor client needs and develop service plans.
• Connect clients to community services and resources and advocate for clients.
• Assist with program enrollment and orientation.
• Coordinate client medical appointments, transportation, housing, money management, etc.
• Develop and assist with client medication management plan.
• Obtain and maintain entitlements for clients.
• Maintain accurate and complete documentation.
• Develop and coordinate transitions plans and program discharge.
