Job summary

Sheppard Pratt is seeking a Service Coordinator for the Residential Rehabilitation program in Baltimore, MD. The role is an on-site, full-time position with a $20.95/hour rate and a Monday–Friday 9am–5pm schedule.

You will assess client needs, build service plans, and connect clients to community resources to help them obtain housing and services. The position requires a high school diploma or GED and at least 1 year of related experience; a bachelor’s degree is preferred.

Qualifications

• A high school diploma or GED is required.
• A bachelor’s degree is preferred.

Responsibilities

• Assess and monitor client needs, develop service plans, connect clients to community services and resources, and advocate for clients as necessary.
• Assist clients through program enrollment and orientation.
• Coordinate client medical appointments, transportation, housing, money management, etc.
• Develop and assist with client medication management plan.
• Obtain and maintain entitlements for clients.
• Maintain accurate and complete documentation.
• Develop and coordinate transitions plans and program discharge.
