Overview
In this Monday-Friday 8am-4pm position, we are seeking an RN to oversee our Adult Medical Day Services program.
Responsibilities
- Ensure that psycho-social needs are assessed, evaluated, and recorded, and that services are provided to meet the identified needs as part of the coordinated care plan.
- Ensure the following program components are available:
- Case management, including health education
- Interdisciplinary care planning
- Nursing services
- Nutrition
- Social services
- Assistance with activities of daily living
- Planned therapeutic or recreational activities
- Pharmaceutical services
Provide social services ensuring registrant psycho-social needs are assessed, evaluated, and recorded, and that services are provided to meet the identified needs as part of the coordinated care plan.Make arrangements for the use of and/or access to other community resources as needed.Ensure a comprehensive assessment is completed upon admission and every six months thereafter per regulation.Maintain a confidential, intact, and readily available clinical record for each registrant.Supervise direct care staff to ensure registrants receive assistance in accordance with their plan of care.Create a Monthly Activity Calendar.Manage staff to address ADLs and programming needs, and document throughout the day.Complete documentation of daily programs, nursing clinical needs, appointments, and ADLs daily.Perform monthly audits on documentation and assessments, and submit these to the administrator.Prepare weekly and monthly reports for the administrator, including information on:- Nursing Clinical Needs
- Social leisure Program participation
- ADLs
- Medical Appointments
- Scheduling of upcoming appointments
- Assessment and documentation completion

Qualifications
- Graduate of an accredited nursing school.
- Current active RN license required.
- At least one year of supervisory experience required.
- This position requires regular interaction with residents, coworkers, visitors, and/or supervisors. To ensure a safe work environment and effective communication, the employee must be able to read, write, speak, and understand English at an intermediate or higher level.
