Overview

In this Monday-Friday 8am-4pm position, we are seeking an RN to oversee our Adult Medical Day Services program.

Salary

$26 / hr - $33.80 / hr

Responsibilities
  1. Ensure psycho-social needs are assessed, evaluated, recorded, and services are provided accordingly as part of the care plan.
  2. Ensure program components are available, including case management, interdisciplinary care planning, nursing services, nutrition, social services, assistance with daily activities, recreational activities, and pharmaceutical services.
  3. Provide social services, ensuring psycho-social needs are assessed, evaluated, recorded, and services are provided accordingly.
  4. Arrange access to community resources as needed.
  5. Complete comprehensive assessments upon admission and every six months.
  6. Maintain confidential, accurate, and accessible clinical records for each registrant.
  7. Supervise direct care staff to ensure assistance aligns with care plans.
  8. Create a monthly activity calendar.
  9. Manage staff to address ADLs and programming, documenting throughout the day.
  10. Document daily programs, clinical needs, appointments, and ADLs.
  11. Perform monthly documentation audits and submit reports to the administrator.
  12. Submit weekly and monthly reports on nursing needs, program participation, ADLs, appointments, and assessments.
Qualifications
  1. Graduate of an accredited nursing school.
  2. Current active RN license.
  3. At least one year of supervisory experience.
  4. Ability to read, write, speak, and understand English at an intermediate or higher level for safe and effective communication.
