Job summary

SCMHCC in Augusta, KS is seeking an Adult Case Manager to deliver medically necessary services in the Community Support Program for adults with serious mental illness and co-occurring substance use disorders. You will apply the Strengths Model and IDDT to guide recovery and client empowerment.

You will conduct Strengths Assessments, co-develop Personal Recovery Plans, and facilitate Crisis Plans, delivering most services in community settings while supporting integration and symptom management.

Qualifications

• BA/BS in human services, psychology, social work, or related field; substitution allowed.
• One year of experience may substitute for one year of education.
• Reliable transportation per transportation requirements.
• Must pass criminal background check and driver’s license review.

Responsibilities

• Develop continuous, supportive working relationships with assigned clients in the Community Support Program.
• Conduct Strengths Assessments, co-develop Personal Recovery Plans, and facilitate Crisis Plans.
• Deliver the majority of direct client services in community and non-office settings.
• Review treatment plans quarterly to monitor progress and update goals.
