Description

The Department of Mental Health (DMH) with the Pocasset Mental Health Center is seeking a talented and experiencedAdult Case Manager (Human Services Coordinator I) to serve as part of the DMH Cape & Islands Case Managementteam. The incumbent to this role will provide assessment, outreach, support, service coordination and person-centeredadvocacy to DMH eligible adults by promoting the agency’s philosophy of recovery, person-centered choices andcultural competence.
The selected candidate will utilize strength-based goal setting and foster independent decision-making for persons served.Additional responsibilities include the development, implementation and monitoring of individual service plans andassisting individuals in securing services while performing all duties to reflect the highest level of respect and dignity forthe persons-served.

Please Note:This position is 40 hours per week. The selected candidate will work Monday through Friday from 8:30am -5:00pm. The days off are Sunday and Saturday.

Duties and Responsibilities (these duties and responsibilities are not all inclusive)

Review eligibility determination record and obtain clinically relevant information through medical and psychiatric records,mental health assessments, school records as well as interviews with clients and service providers.

Develop Comprehensive Assessment and ISP for Persons Served, reviewing treatment progress regularly and makingmodifications to the ISP as necessary.

Schedule multidisciplinary team meeting to formulate recommendations for treatment and service and maintainconsistent and effective collaboration and communication with service providers and colleagues.

Maintain regular contact with and communicate relevant information to clients, service providers, legal guardians and family members as appropriate and makes appropriate referrals when necessary.

Ensure that updated collateral information (for example, mental health assessments and legal documents) are included inclient chart and notes are written on all significant contacts, including but not limited to, face to face visits, phone contactswith the client’s family/guardian (including contact relating to ISP content), incidents involving a client reported from athird party, all crisis events.

Maintain both electronic and paper medical records with up to date information, reflecting current bio-psych-socialinformation on persons served.

Participate in meetings, case conferences

Required Qualifications

Knowledge of psychosocial problems of those with mental/emotional disorders and the multiple handicapped.

Demonstrated skill in the principles, practices and techniques of counseling, interviewing, motivation and reinforcement.

Experience with the types and symptoms of mental/emotional disorders and the programs and services available to thispopulation.

Expertise in the methods of general report writing.

Knowledge of principles and practices of crisis intervention and group process for counseling the mentally ill.

Ability to prioritize and multitask duties based on client need, team and service needs.

Preferred Qualifications

3+ years of experience in behavioral health services.

Special Requirements

Travel is required - This position directly manages cases in the DMH Cape Cod and Islands Site catchment areas and services in which will be primary travel locations; however, frequent travel is required throughout the DMH Southeast Area and occasional required statewide travel to various acute inpatient hospitals or other treatment settings. Current and valid Massachusetts Class D (or equivalent from another state) Motor Vehicle Operators license required.

About the Cape Cod and the Islands Community Mental Health Center

The CC&I CMHC provides comprehensive services to adults and children. Programs include a 16-bed acute in-patient unit, Case Management, Day Treatment Program, and Adult Community Clinical Services (ACCS). The CC&I CMHC is a Joint Commission certified facility. Qualifications

Applicants must have (A) at least three (3) years of full-time or equivalent part-time, professional experience in human services work or social work or (B) any equivalent combination of the required experience and substitutions below.

Substitutions:

I. A Bachelor's degree or higher in a related field may substitute for two (2) years of the required (A) experience.

Based on assignment, travel may be required. Incumbents who elect to use a motor vehicle for travel must have a current and valid motor vehicle driver's license at a class level specific to assignment.

Based on assignment as Qualified Intellectual Disabled Professionals, within the Department of Developmental Services, a Bachelor's degree or higher in social work, psychology, sociology, counseling, counseling education, education of the physically or emotionally handicapped, education of the multiple handicapped, education of the learning disabled, human services, rehabilitation, rehabilitation counseling, nursing, recreation therapy, art therapy, dance therapy, music therapy, physical education or other a related field is required.

Salary placement is determined by a combination of factors, including the candidate’s years of directly related experience and education, and alignment with our internal compensation structure as set forth by the Human Resources Division’s Hiring Guidelines. For all bargaining unit positions (non-management), compensation is subject to the salary provisions outlined in the applicable collective bargaining agreement and will apply to placement within the appropriate salary range.
