Job summary

Assistance Plus is hiring an Adult Case Manager in Newburgh, ME 04444 to support adults with behavioral health needs. You will develop client-centered service plans and coordinate services through the ASO Managed Care System, traveling within your designated area to provide needed support.

The role runs weekdays with a 6am–5pm window and offers mileage reimbursement, paid travel time, holidays, and a full benefits package including medical, dental, and voluntary insurances, plus life insurance

Qualifications

• Proof of vehicle insurance required ($50K/$100K)
• Valid Maine driver’s license required
• High school diploma or equivalent required
• MHRT/C or MHRT/CP required
• Intermediate level computer experience required

Responsibilities

• Provide services to adults with behavioral health diagnoses to develop client-centered service plans.
• Coordinate services approved through the ASO Managed Care System.
• Maintain current benefit and financial information for clients to ensure benefits remain applicable.
