New Alternatives for Children, Inc. (NAC) in New York City seeks a Social Worker/Case Planner to join the Adoption and Kinship Support Program. You will provide family-centered services, conduct psychosocial assessments, and coordinate care with a multidisciplinary team.

The role requires a Master’s in Social Work (preferred) or related degree with case management experience; bilingual Spanish is a plus and may affect salary. Some evenings and travel are required.

Qualifications

  • Masters in Social Work preferred.
  • Bachelor's level degree in social work, psychology, or human service.
  • Bachelor's level degree in any field with at least two years of case management experience.
  • General knowledge of adoptive and kinship families.

Responsibilities

  • Provides supportive services to adoptive and kinship families post-finalization to strengthen and maintain placements and prevent disruptions/dissolutions.
  • Provides family-centered needs assessment, psychosocial, trauma-screening, case management, and clinical services.
  • Formulates and implements individualized service plans to identify family goals and needs.
  • Support and monitor permanency to assure safety and needs of the family are met.
  • Conducts monthly home and community-based visits to families.
  • Facilitates monthly support groups and workshops for parents and youth.
  • Coordinates services with NAC's interdisciplinary team.
  • Provides case coordination with hospitals, schools, mental health clinics and community-based organizations.
  • Provides information, referral, and advocacy services.
  • Attends outreach events and assists with recruitment; collaborates with community providers to increase awareness.
  • Attends weekly supervision, monthly staff meetings, case conferences, trainings and workshops.
  • Ensures adherence to policies and completes documentation in our behavioral health record.
  • Note: some evenings work and travel for visits may be required.
