New Alternatives for Children, Inc. (NAC) seeks a Social Worker/Case Planner for the Adoption and Kinship Support Program in New York City.

This full-time role involves providing supportive services to adoptive and kinship families, conducting needs assessments, and coordinating with an interdisciplinary team to prevent disruptions and support permanency. Responsibilities include home visits, group workshops, and ongoing case management, with flexible evening hours and travel as required.

Qualifications

• Masters in Social Work preferred.
• Bachelor's level degree in social work, psychology, or human service.
• Bachelor's level degree in any field with at least two years of case management experience.
• General knowledge of adoptive and kinship families.

Responsibilities

• Provides supportive services to adoptive and kinship families post-finalization to strengthen and maintain placements and prevent disruptions/dissolutions
• Provides family-centered needs assessment, psychosocial, trauma-screening, case management, and clinical services using individual, family, and group work modalities
• Forms and implements individualized service plans to identify family goals and needs
• Supports and monitors permanency to assure safety and needs of the family
• Conducts monthly home and community-based visits to families
• Facilitates monthly support groups and workshops for parents and youth
• Coordinates services as recommended by NAC's interdisciplinary treatment team
• Provides case coordination with hospitals, schools, mental health clinics and community-based organizations
• Provides information, referral, and advocacy services
• Attends outreach events and assists with recruitment; collaborates with community providers to increase kinship/adoption resources
• Attends weekly supervision and monthly staff meetings; participates in trainings/workshops
• Ensures adherence to policies; completes documentation in behavioral health record
• Note: some evenings and travel for home/community visits may be required

(NAC) is an award-winning not-for-profit agency dedicated to serving children and families with medical complexities, chronic illnesses, and disabilities throughout the five boroughs. To assist in achieving this mission, NAC is currently seeking a Social Worker/Case Planner to join its Adoption and Kinship Support Program

NAC Adoption and Kinship Support programs offer families formed through adoption or guardianship a place to find the support, advocacy, information, and a range of professional health and social services needed to help them create or maintain a successful and permanent home. Our goals are to prevent post-adoptive and post-guardianship dissolutions/disruptions, provide assistance to families so that children may be cared for in their own homes with their adoptive parent(s) or legal guardian(s), and strengthen post-adoptive and post-guardianship families.

• Salary Range: Master's Level- $60,000-$65,000 | Bachelor's Level- $50,000-$55,000

• Formulates and implements individualized service plans to identify family goals and needs
• Support and monitor permanency to assure the safety and needs of the family are being met
• Conducts monthly home and community-based visits to families
• Facilitates monthly support groups and workshops for parents and youth
• Coordinates services as recommended by NAC's interdisciplinary treatment team, which includes our psychiatrist, psychologist, pediatrician, nurses, educators, parent advocate, and resource staff
• Provides case coordination with hospitals, schools, mental health clinics and community-based organizations
• Provides information, referral, and advocacy services
• Attends outreach events and assists with recruitment efforts; collaborates with community providers to increase awareness about kinship and adoption resources
• Attends weekly supervision, monthly staff meetings, case conferences, trainings and workshops
• Ensures adherence to agency and programmatic policies and regulations; completes all required documentation in our behavioral health record.
• Please note the flexibility to work some evenings. Travel required for home and community visits.

Required Education and Experience

• Masters in Social Work Preferred
• Bachelor's level degree in social work, psychology, or human service
• Bachelor's level degree in any field with at least two years of case management experience
• General knowledge of adoptive and kinship families
