Job summary

Stevens Treatment Programs in Swansea, MA is seeking a Licensed Practical Nurse (LPN) to join our Health Department serving youth in residential and congregate care settings.

Under the Nurse Manager, the LPN provides nursing support, health screenings, education, medication administration, and back-up on-call coverage. A MA LPN license and at least two years post-licensure experience are required.

Qualifications

• Current Massachusetts LPN license.
• Minimum of two years post-licensure experience.
• Experience in community health, social services, or adolescent care preferred.
• Strong clinical assessment, communication, and organizational skills.
• Ability to work independently while collaborating with an interdisciplinary team.
• Experience or comfort working with adolescents with mental health or behavioral challenges.
• Valid driver’s license and acceptable driving record.

Responsibilities

• Assess and respond to youth health needs, including health screenings, monitoring, and follow-up care.
• Provide health education, coaching, and wellness support to youth, including Healthy Eating Plans.
• Support medication administration, health-related training, and other Health Department activities.
• Provide backup on-call nursing coverage during Nurse Manager absences.
