Oak Plains Academy, a UHS company, seeks a dynamic Case Manager (MSW) to join a comprehensive residential treatment program for adolescents. You will deliver quality case management, admission support, and utilization review within a collaborative treatment team dedicated to trauma healing and resiliency.

The role requires a Master’s in Social Work, licensure eligibility, and 1–3 years in psychiatric behavioral health; strong crisis and behavior management skills are essential.

Qualifications

• Master’s degree in Social Work required.
• Licensed, LPC, LCSW or LMFT or eligibility for licensure track in social work or related field acceptable.
• 1–3 years experience with psychiatric behavioral health population; 3–5 years preferred.
• Background check, fingerprinting and employment verification required.
• Valid Driver's License.

Responsibilities

• Provide quality case management, admission support, utilization review activities and services to residents, their families and care providers.
• Serve as a member of the treatment team to support the organization’s treatment program and philosophy and ensure quality treatment.
• Knowledge of case management, discharge planning, age-specific growth and development, crisis and behavior management, and therapy concepts.
