Job summary

Northpoint Recovery Holdings, LLC is seeking a Care Coordinator for Imagine Omaha (Adolescent outpatient) to support clients and families by coordinating resources, co-facilitating group sessions, and assisting with educational needs. The role requires supervising clients, maintaining programming schedules, and applying de-escalation techniques within a multidisciplinary team.

Qualifications

• Bachelor’s Degree in relevant field or commensurate experience.
• Must be at least twenty-one (21) years of age.
• 1+ year of relevant work experience.
• Knowledge of English usage and excellent communication skills.
• Knowledge of surrounding geographic area to ensure effective routing.
• Ability to organize and prioritize competing priorities.
• Proficient in patient rights, safety and sanitation.

Responsibilities

• Interact with clients, observe behaviors, and report observations to clinical staff.
• Follow reporting guidelines with staff, clients, and guardians.
• Monitor progress on goals and revise programming as needed.
• Ensure facility cleanliness and upkeep under leadership.
• Assist clients with scheduling community appointments.
• Collaborate with onsite educators and school resources.
• Provide facilitation and co-facilitation of group sessions.
• Oversee safe transport of clients when necessary.
• Maintain route and maintenance records for leadership.
• Assist in orientation of new admissions.
• Model professional attitude and communication for clients and staff.
• Utilize therapeutic communication and maintain boundaries.
• Assist with treatment and discharge planning.
• Document information regarding client care per policies.
• Use de-escalation to maintain safety for all.
• Respond to emergencies with counseling and administrative staff.
• Maintain confidentiality and adhere to policies.
• Adhere to all Company policies and perform other duties.
