Job summary

Community Care Of West Virginia Inc. seeks a School-Based Behavioral Health Therapist to provide psychosocial assessment and develop treatment plans for adolescents and adults in a school-based setting, under clinical supervision.

The role emphasizes engagement, education, and coordination of care to meet mental health and substance use needs. The therapist will deliver evidence-based interventions, maintain thorough documentation, and collaborate with treatment teams to improve patient outcomes

Qualifications

• Master degree in social work (MSW) from a CSWE-accredited program.
• Licensed by the State Board of Social Work.
• Two years of supervised social work experience in a health care setting, with preference for chemical dependency experience.

Responsibilities

• Engage patients and families in active care planning to address behavioral, clinical, social, and environmental needs.
• Provide clinical or psychotherapeutic services under supervision, including individual, family, and group modalities.
• Assess and educate patients on their behavioral health condition and self-management strategies.
• Provide direct care management to high-risk populations and consult with care team members.
• Document treatment goals, progress notes, and billing per standards.
