By The Bay Health, a nonprofit hospice, seeks an Admissions/Team Nurse to support Nurse Case Managers, coordinate admissions for eligible hospice patients, and ensure seamless patient care across San Francisco and Marin counties.
The position requires an Associate’s or Bachelor’s degree in nursing, California RN license, and experience in acute care, home health, or hospice; you will perform in-person visits, use the nursing process, and collaborate with a transdisciplinary team.

The Admissions/Team Nurse provides coverage for Nurse Case Managers to cover patient/staffing needs and processes admissions for eligible hospice patients in coordination with Team Manager.
Monday – Friday; 8:30am – 5:00pm
San Francisco County with additional coverage to Marin County as needed
