Job summary

Hope Hospice in Pleasanton, CA is seeking an Admissions/Team Nurse to support Nurse Case Managers and manage admissions for eligible hospice patients in coordination with Team Manager.

The role requires active nursing licensure in California with ASN or BSN, and experience in hospice or acute care settings is preferred. The schedule is Mon-Fri, 8:30am–5:00pm, with a focus on coordinated patient care.

Qualifications

• ASN or BSN in nursing from an accredited School of Nursing.
• Experience in acute care/home health/hospice setting preferred.
• Public Health Nursing certification desirable.
• HPNA certification preferred.

Responsibilities

• Make in-person nursing visits to patients as assigned.

• Provide effective management of symptoms with escalation as needed.

The Admissions/Team Nurse provides coverage for Nurse Case Managers to cover patient/staffing needs and processes admissions for eligible hospice patients in coordination with Team Manager.

Essential Duties & Responsibilities

• Make in-person nursing visits to patients as assigned, or as indicated when case managing.
• Use the nursing process to assess, plan, intervene and evaluate following patient/family needs and goals.
• Provide effective management of symptoms, including prompt escalation to providers or others as indicated.
• Coordinate patient/family services and prioritization of needs with the members of the transdisciplinary team or outside providers/services.
• Make telephone contact and/or conduct assessment visits with potential hospice patients to provide information on agency services and assess patient status.

Education/Experience

• ASN or Bachelor’s degree in nursing from an accredited School of Nursing.
• Experience in acute care/ home health and/or hospice setting preferred.

Certificates, Licenses, Registrations

• Current license as a registered professional nurse in the State of California.
• Public Health Nursing certification highly desirable.
• HPNA certification preferred.

Pay Range

$58.34 - $72.95 hourly
