Job summary

Hope Hospice in Pleasanton, CA is seeking an Admissions/Team Nurse to support Nurse Case Managers and coordinate admissions for eligible hospice patients. The role involves nursing assessments, care planning, and collaboration with the transdisciplinary team to ensure timely patient services.

Requires ASN or BSN, CA RN license, and relevant experience; Public Health Nursing or HPNA certifications are desirable. Schedule is Monday–Friday, 8:30am–5:00pm.

Qualifications

• ASN or Bachelor’s degree in nursing from an accredited School of Nursing.
• Current license as a registered professional nurse in the State of California.
• Public Health Nursing certification highly desirable.
• HPNA certification preferred.
• Experience in acute care/ home health and/or hospice setting preferred.

Responsibilities

• Make in-person nursing visits to patients as assigned, or as indicated when case managing.
• Use the nursing process to assess, plan, intervene and evaluate following patient/family needs and goals.
• Provide effective management of symptoms, including prompt escalation to providers or others as indicated.
• Coordinate patient/family services and prioritization of needs with the members of the transdisciplinary team or outside providers/services.
• Make telephone contact and/or conduct assessment visits with potential hospice patients to provide information on agency services and assess patient status.

Pay Range

$58.34 - $72.95 hourly
